The purpose of an adult needs assessment is to identify the type of need, the outcomes the personal would like to achieve, the risks, strengths and recommendations.
Every person's circumstances are different, so the level of assessment required will depend on the complexity of their care and support needs. Following an initial discussion, I will recommend the most appropriate assessment and provide a clear fee before any work begins.
This service may include:
• Comprehensive strengths-based assessment
• Professional written report
• Personalised recommendations
• Sign posting to appropriate services
• Follow-up telephone consultation
* Assessments can be completed in a person's home, a care home, in hospital or in an
  interim bed following discharge from hospital such as Discharge to Assess (D2A).
* A personalised care and support plan can be added to your assessment at a discounted
  fee if you would like practical guidance on how your assessed needs should be met.

• Follow-up consultation

NHS Continuing Healthcare (CHC) Support
Navigating the NHS Continuing Healthcare process can be complex, challenging and overwhelming. With an in depth understanding of both the Care Act 2014 and NHS Continuing Healthcare; I provide independent, expert support throughout every stage of the process, helping individuals and families understand their rights, prepare strong evidence and ensure assessments are fair, thorough and person-centred.

Unlike many services, you'll work directly with the same experienced Social Work throughout, providing continuity, reassurance and a consistent point of contact.

• Monitoring changing care needs
